    Preferential tariff rates will only be accepted for products that have been submitted by the Vendor and meet all of the following terms. There will be no exceptions afforded.

 

1. Certification is on an LCBO-approved Certification of Origin template Declaration Form.

 

2. Certification Form(s) submitted in original PDF format submitted by email.

 

3. Maximum of 5 products per Vendor # on a PDF Declaration Form.

 

4. Submissions exceeding 5 SKUs must be accompanied by an LCBO Continuation Excel Worksheet.

 

5. Active LCBO-registered Products with accurate Vendor/Supplier and Product Numbers.

 

6. Submitted via "Submit" Form button on the LCBO Template Form and no changes made to the prepopulated submission email
